Updates to Our Practice Policies for 2023 and Update Your Contact Information

We want to take a minute to update you on policy changes that will be taking effect in January. The goal of these policy changes are to make it easier for you to know what to expect when scheduling an appointment. Please download the policy forms and return them on your next visit, or upload them through your patient portal.

We’re also updating the contact information for each patient as we enter the New Year.

Easy Access to Your Patient Portal

Your private patient portal is the preferred location for all patient communications and prescription refill requests. You’ll have access to your test reports and other personalized information. This is where you may also upload your Current Profile Update and other important documantation.
